Online appointment scheduling is provided through a scheduler from CharmHealth, the practice’s electronic health record (EHR) provider, embedded on the home page. Information you enter there — such as your name, date of birth, contact details, and the reason for your visit — goes directly to the practice’s EHR and is handled under the practice’s Notice of Privacy Practices and applicable healthcare privacy laws. It is not stored by this website.

How it’s used

Information you enter in the online scheduler is used to schedule and confirm your visit. It is not sold, shared for marketing, or added to mailing lists.

Cookies, analytics, and embedded content

This site sets no advertising or tracking pixels. It may use a basic analytics tool to understand general visit patterns (pages viewed, approximate region); no health information is involved. The map and the online appointment scheduler on the home page are provided by third-party services, which may set their own cookies when they load. Links to outside sites — such as the FMCSA National Registry — are governed by those sites’ own policies.

For a medical emergency, call 911. This website, including its online scheduler and email, is not monitored around the clock and is not for urgent medical needs.
